Every provider needs outcome data. Every founder is tired of surveys. Both things are true, and the usual response is to send more surveys.
Collect once, share many times
The same founder is often asked the same five questions by four organizations in one quarter. A shared intake and a shared record removes three of those asks without removing any of the data.
Attest at the moment of value
Ask at the point where something actually happened: after a loan closes, after a contract is won, after a workshop produces a deliverable. A single question asked at the right moment beats a twelve-question form sent in January.
Prefer observed data over reported data
Registrations, attendance, referrals completed, and capital deployed are already recorded somewhere. Use those first, and reserve founder time for the things only a founder can tell you: confidence, clarity, and whether the help arrived when it was needed.
What funders should ask for
Ask for outcome density, not survey volume. The question is not how many founders responded. It is how many founders moved, and what moved them.
